One of the tools for change is the introduction of a care coordinator, who is not directly involved in the day-to-day delivery of care, but helps to organise and streamline it.

Job description:
- Coordination of patient care in the field, both within the hospitals/providers under the AKESO holding and with external entities.
- Collaborating with physicians and other healthcare professionals within AKESO Holding (and external) to provide the most effective patient care.
- Prepare/compile medical records from various examinations to support the decisions of the multidisciplinary team, both from external and internal entities.
- Participate in the creation of patient pathways and their optimization.
- Communicate with patients to address their needs and assist them in navigating through the healthcare system.
- Participate in the organizational and administrative support of the department he/she represents and the medical staff.
WHAT WE REQUIRE FROM YOU
Professional requirements:
- University degree (min. Bc)
- Min. 10 years of experience, ideally in healthcare (experience in healthcare is not a requirement, we can train you). In case of strong interest and/or relevant healthcare experience, 5 years of experience is possible.
- Proficiency in basic office software (outlook, word, excel).
- Ideally an overview of diagnostic methods in healthcare (not a prerequisite).
Other requirements:
- Active approach to your work.
- Ability to have fun (listen/collaborate) with a wide range of professions and personalities (healthcare professionals, patients and management)
- ability to build long-term relationships based on trust and reliability
- Reliability in working, meeting deadlines and completing tasks
- proactive approach to dealing with patient situations
